Johns Creek CARES Business Grant Program

JC Cares Business Grant Program Johns Creek businesses that have suffered significant, temporary loss of revenue or job losses directly related to the COVID-19 pandemic, and businesses that have been directly affected by the Governor’s Executive Orders for closure, are eligible.

Section 1. Confirmation of Eligibility

Applicant must check box to confirm that each statement is true for all questions. If any statement is not true, the business is not eligible.

1. Business was in operation in Johns Creek in 2020 and during the COVID-19 pandemic and can provide acceptable proof of operation in 2020 and during the COVID-19 pandemic. Acceptable proof includes a 2020 Johns Creek business license.

2. Business is currently licensed in Johns Creek and can provide acceptable proof of licensure. Acceptable proof includes a 2020 Johns Creek business license for which all applicable fees have been paid.

3. Business is in good standing and is not subject to any compliance issues or active judicial proceedings. No owner, partner, or principal actor of the business has a felony conviction within the last five years for which he or she is still service a sentence (including prison, parole, and probation). Business does not have any current city, county, state, or federal tax lien. Business does not have any legal actions against or from the City of Johns Creek and is not operating in violation of any state, federal or local laws.

4. Business is currently in operation.

5. Business has less than 250 employees.

6. Business has a physical commercial location in Johns Creek and if incorporated, is incorporated by a Georgia corporation.

7. Business is not based in a residential home.

8. Business is public-interfacing (meaning allows walk-in, non-appointment based interactions).

9. Business experienced disruption and was negatively impacted as a result of the COVID-19 pandemic.

Examples of business disruption include mandated closure (as a result of the Governor’s Executive Order), partial closure (as a result of the Governor’s Executive Order), voluntary closures to promote social distancing, or decreased customer demand.

Individuals performing service to administer the City of Johns Creek CARES Business Grant Program are authorized to make all the inquiries that they deem necessary to verify the accuracy of the information contained herein. Additionally, applicant agrees that in the event that money is provided pursuant to this application, the City or its agent shall be entitled to access and audit such records as may be necessary to prevent fraud in this process or ensure compliance with federal requirements.

Under penalties of perjury, I declare that I have read the foregoing application and that the facts stated in it are true. I understand that knowingly making a false written declaration, I will be required to refund and any all grant funds awarded to me and/or my business.

Documents submitted to Georgia governments and agencies are subject to the Georgia Open Records law (O.C.G.A. § 50-18-71 et. seq.). The Georgia open record law has certain exceptions where disclosure is limited or not required. All information in the submitted Application, including attachments, may not have to be disclosed (See O.C.G.A. § 50-18-72(a)(20)(A through E) or if O.C.G.A. § 50-18-72(a)(34) is complied with); however, Applicant understands that the City of Johns Creek, Georgia and those administering the City of Johns Creek CARES Business Grant Program cannot warrant, represent, or guarantee the applicability of any such exception.
