Please submit this form to participate in the "15th Annual Founders Day Parade, Kickoff to the 2021 North Fulton Holiday Season," Saturday, Dec. 3, 2022 at 10 a.m. along Lakefield Drive and Johns Creek Parkway.
This year's parade theme is "Happy Holidays!" The entry deadline is Tuesday, Nov. 1, 2022.

NOTE: ALL ENTRIES WILL BE INSPECTED THE DAY OF THE PARADE.
In consideration of this entry, our organization accepts full responsibility for the group and/or float entered and waives all claims against the City of Johns Creek, Fulton County and Parade Sponsors for theft, damage, any unforeseen incident, illness or injury resulting from participation in the Parade. I also give permission for the use of our name and picture in any media coverage. I understand that the City of Johns Creek and sponsors of the Parade cannot be held responsible for theft, damage, accident, injury OR death. Organizers reserve the right to cancel event due to hazardous weather conditions.

Liability Waiver
“I have read the application and liability waiver carefully. In consideration of the benefits received, I understand and appreciate the risks that are inherent in using City of Johns Creek (“COJC”) facilities and/or participating in COJC programs, and hereby assume all risks of illness, including death, that I may sustain while participating in or as a result of, or in any way growing out of the use of COJC facilities and/or the participation in COJC programs. I hereby assert that my use and/or participation is voluntary and that I knowingly assume all such risks. I hereby agree that for the sole consideration of the City of Johns Creek allowing me to use COJC facilities and/or participate in COJC programs, I do hereby release and forever discharge the City of Johns Creek, and its officers, agents and employees of any and from all claims, demands, rights and causes of action of whatever kind or nature, arising from and by reason of any and all known and unknown, foreseen and unforeseen illness, and the consequences thereof, resulting from or an any way connected to any use of COJC facilities and/or participation in COJC programs.”
