Waiver of Liability: The participant will indemnify and save harmless the City of Alpharetta and/or the Alpharetta Recreation and Parks Department, the City of Roswell and/or the Roswell Recreation Parks, Historic and Cultural Affairs Department, Alpharetta YMCA/ Ed Isackson, City of Johns Creek and/or Johns Creek Recreation and Parks Division, Milton Parks and Recreation Department, the organizers of the activity, sponsors, hosting facilities and its employees, supervisors, and any and all of them, all of which are hereinafter referred to as “releases” from and against all claims, damages, actions, judgments, decrees, penalties, and/or liability and expenses in connection with loss of life, bodily and/or personal injury caused or alleged to be caused in whole or in part by the negligence of the release or otherwise. I have prepared myself for the events which I entered by practicing prior to the games/activities. To the best of my knowledge and belief, I have no physical restrictions which should prohibit my participation in the games/activities. The undersigned has read the above waiver/release, understands that they have given up substantial rights by signing it, and signs it voluntarily. I also hereby grant permission to the organizers to use for any official purpose any photographs, videotapes, recordings or any other records of program activities depicting myself or my minor child. Note: I have carefully read, understand and agree to the policies as stated above.
